All I did to this form was to add the "UID 600 - Supply Kind" to the drop down menu.

It will allow the addition of a 'dropped weapons' kind, which is require for some of the work being done in Canada at the moment.

I have added the updated change request form to the svn repository.

Could you please add this file to the digital library.
