Since some of the questions are being reworded and others combined, I suggest a new survey is created for release.

Also, make sure you have actively selected the settings you want from the settings gear wheel.

I recommend checking the boxes:

"Collect email addresses -- Response receipts -- Always"

"Limit to 1 response"

"Edit after submit"

"See summary charts and text responses"
